Welcome to our online PrEP questionnaire, which is a part of every entrance examination at our centre. It allows you to answer all important questions which will help us determine, whether PrEP is the right choice for you. All questionnaires are reviewed and evaluated by a doctor.
The system is completely anonymous. Your answers will become a part of your medical file which are confidential.
The only way to trace your answers is through your unique 4-digit identification code (in the format PREPxxxx). It will be generated after finishing the questionnaire. Please save it as a screenshot on your phone, the nurse will need it to book an appointment.
